At Oaklea we will work with you to help you achieve this whilst taking into account other needs you may have which affect you achieving your goals.

A Lifestyle Co-ordinator will look at all aspects of your life with you. These include:
  • Homelife- family, friends, relationships
  • Employment, education and training
  • Leisure and free-time
  • Community, participation and involvement in your neighbourhood
All of these contribute to independent living.

You may require support to help with your independent living. From full time care to supported living, the Oaklea Trust aims to support you to be a fully participating citizen in society.

Throughout our involvement we will work with you to maintain and promote (or, in some situations, re-establish) positive family relationships. One-to-one mentoring is a strong ingredient in the support 'mix' where we can identify and agree your aspirational needs and set realistic goals to work towards.

The adult care provision you receive is a life-long commitment and the Oaklea Trust are ready to support you and other adults of any age and deliver the right level of ongoing care.
