Get Support

Whether you are a self-funder or referred to us from a third party (such as your Local Authority Care Manager or Job Centre Disability Employment Adviser) you can expect a warm welcome from a member of the Oaklea team.

Initially, we will meet with you and those people who are important to you to find out how we can support you and together, by using our own 'My Life My Choices' approach, help agree your support plan.

My Life My Choices shows how the Oaklea Trust supports each customer; we consider it to be hugely important as it is our tool to support customers towards their independence.

There are four key elements to My Life My Choices:
  • All About Me
  • Staying Safe
  • My Goals and Outcomes
  • My Other Plans
We will give you a customer agreement that tells you what you can expect from us, how much your support costs and what rights you have if you are not happy with any aspect of the services we provide.

Our team of Lifestyle Coordinators will work with you to help achieve your goals and we will also offer you advice and guidance about how to access individual funding to help pay for your care and support.

If you would like the Oaklea Trust to look after your care and support for you, please ask one of our team about Individual Service Funds or Personal Budgets.

If you want to select your own personal assistants or helpers to care for you, please ask about Right Focus.

When you first start receiving support from Oaklea, we guarantee to offer you a named contact so you will always have a member of the team that is assigned to you. We will also try to offer you continuity so that once you are happy with your Lifestyle Coordinator/Employment Adviser you can maintain that relationship going forward.
